How do I become a member?<\/mark><\/h2>\n\n\n\n

All notaries registered with the Roll of the Chambre des notaires du Qu\u00e9bec<\/em> who have signed the consent form<\/a> have access to the Programme d\u2019aide aux notaires<\/em> (PAN), as do their family members*. Once the form has been completed and the registration request received, the notary will receive an email from TELUS Health inviting them to create an account. Family members* may then be added to the account.
Please note that a delay of several weeks should be expected following the signing of the consent form before access to services is activated, as registration lists are submitted only twice a year, in January and June.<\/strong>
To obtain quicker access to services, we encourage you to contact us directly<\/strong> after signing the form.<\/p>\n\n\n\n

Students enrolled in the Master of Notarial Law program, as well as candidates to the profession, also have access to many PAN services. They will receive an activation code through their university and may create their account via the TELUS Health One mobile application.<\/p>\n\n\n\n

In addition, notarial firms are encouraged to annually enroll their non-notary employees in the Employee Assistance Program (EAP) for notarial firms directly through TELUS Health (see details below). This service provides access to virtual care (telemedicine) and mental health support at a minimal cost.<\/p>\n\n\n\n

*Family: spouse or common-law partner, and dependent children aged 25 or under residing at the same permanent address as the eligible person.<\/em><\/p>\n\n\n\n

Confidentiality<\/mark><\/h2>\n\n\n\n

Confidentiality is one of the main characteristics of this assistance program.<\/strong><\/strong>\u202fConfidentiality is further protected by the fact that these services are provided by professional health workers held to the strictest professional confidence under Canadian law and their respective professional codes of conduct. Any contact, information or conversation between the concerned parties remains strictly confidential and without prejudice. As such, no information can be disclosed without the written authorization of the individual using the program.<\/p>\n<\/div><\/div>\n<\/div><\/div>\n\n\n\n\n

Enrollment in the mental health services plan: $3.34 \/ month \/ employee<\/p>\n\n\n\n

Enrollment in the virtual care (telemedicine) services plan: $4.51 \/ month \/ employee<\/p>\n<\/div><\/div>\n<\/div><\/div>\n\n\n\n\n

Register your NON-NOTARY employees for the program<\/h2>\n\n\n\n

TELUS Health is now the service provider for the Employee Assistance Program (EAP) offered through the Programme d\u2019aide aux notaires<\/em> (PAN). You must now register your employees directly with TELUS Health so they may benefit from the full range of services available.<\/p>\n\n\n\n

1. Complete Form 1<\/strong>: Form 1<\/strong><\/a>
You must complete \u201cForm 1 \u2013 Employee Enrollment in the EAP\u201d.
\u2022 All fields must be completed.
\u2022 The HR and Billing contacts may be the same person, including yourself. In that case, these sections do not need to be completed separately.
\u2022 Please do not modify the pre-filled information, except for the \u201cYes or No\u201d field.<\/p>\n\n\n\n

2. Complete Form 2<\/strong>: Form 2<\/strong><\/a>
You must complete \u201cForm 2 \u2013 Employee List for the EAP\u201d (all fields must be completed).<\/p>\n\n\n\n

3. Complete Form 3<\/strong>: Form 3<\/strong><\/a>
You must complete \u201cForm 3 \u2013 Pre-Authorized Payment\u201d.
\u2022 All fields must be completed, except for the \u201cBilling Account Number \/ Client Code\u201d field, as you will not yet have this information.<\/p>\n\n\n\n

4. Submit the 3 Forms<\/strong>
You must send the three (3) completed forms by email to michelle.soury@telus.com<\/a> (designated representative at TELUS Health) and copy (cc) info@programmepan.ca<\/a>.
Including PAN in copy of your email simply allows us to know that you have completed the registration process for your employees. Since registration is handled directly between your firm and TELUS Health, this is the only way for us to be informed of your enrollment. This also allows us to provide you with promotional materials, such as brochures customized for your firm, intended for your employees and future employees.<\/em><\/p>\n\n\n\n

5. Finalizing the Registration<\/strong>
You will be contacted directly by TELUS Health to complete the final steps of the registration process.
Your employees\u2019 enrollment will become effective within a minimum of three (3) weeks, or on the date indicated in \u201cForm 1 \u2013 Employee Enrollment in the EAP\u201d. Once enrollment is active, each employee will receive an email at the address provided and will be able to activate their account through the TELUS Health mobile application so they may access the services.
